Type 2 Diabetes
Type 1 Diabetes
A local resource to help you understand and
effectively manage your condition
Type 2 Diabetes
A healthy lifestyle and education are two of the best ways to prevent Pre-diabetes developing into diabetes and managing Type 2 diabetes, alongside support from your GP practice.
Understanding what helps and what doesn’t and being able to put the changes into action are keys to remaining happy and healthy.
The Reach for Health Centre has a wealth of experience in supporting people to change their lifestyle so that they can live with their chronic condition rather than letting it rule their life. Our qualified practitioners understand how diabetes can affect you and which changes are the most effective.
Research repeatedly shows that people who are active, eat healthily and understand their condition develop far fewer problems associated with their diabetes and in the case of Type 2 can occasionally reverse the condition.
Not developing diabetes after a diagnosis of pre-diabetes is within your control and we can help you at The Reach for Health Centre get that control with our programme to support you.
Supporting people to live healthy active lives
Our newly opened, purpose built Centre has a wide range of specialist equipment, supporting people to rehabilitate from both acute and chronic problems. Rehabilitation coaches develop bespoke programmes with clients, which can involve both one to one and at times two to one help. The Centre also supports people to live a healthy, active and fulfilled life, developing programmes to help with weight, mental health and long-term conditions to become more active.
There is ample car parking available and easy wheelchair access throughout the Centre. With experience of working with multiple authorities for many years we recognise the need for flexibility in our approach and are proud that we achieve this.
At The Reach for Health Centre we are working with local GP practices to support your care.
Diabetes the potential need for Podiatry
Over 4 million people in the UK have a diagnosis of diabetes.
Having diabetes means that you are at much greater risk of developing foot problems.
This is because raised blood glucose levels can cause damage to the nerves in your feet which may lead to a partial or total loss of sensation in the feet. It can also cause damage to your circulation resulting in less blood supply to your feet. Both complications are risk factors for developing cuts and sores that are difficult to heal. Without early detection and treatment these sores may deteriorate into ulcers and serious infections that can ultimately lead to amputation.
The good news is that most diabetic foot problems can be prevented with an annual foot assessment to monitor your foot health, make you aware of any risk factors and deal with any complications. Regular foot care and patient education helps to empower you and prevent complications. Specialist podiatrists at Elite Feet have undergone extensive postgraduate training to help you manage your foot health with diabetes.
