Medical Referrals
Referring patients to Reach for Health is a straightforward process for medical professionals. Although we are an independent charity and receive no NHS funding, we are proud to be the only non-NHS organisation in Northamptonshire granted an NHS.net email address for referrals by NENE Clinical Commissioning Group (CCG).
Our referral form is available on the NENE CCG portal and can also be downloaded from this page. Completed forms should be emailed to: nccg.reachforhealthreferrals@nhs.net.
Our Daventry centre provides specialist rehabilitation support for patients recovering from major health trauma, as well as those with long-term and chronic health conditions. We also accept referrals for a wide range of general health issues including:

Over 90% of our users are referred by GPs, NHS hospital doctors, consultants, and other healthcare professionals. Many of our patients arrive using wheelchairs or walking aids, which is uncommon in most other rehabilitation centres in the region.
We are proud to be the only centre in Northamptonshire to have signed the Armed Forces Covenant, offering dedicated recovery services for both physical and mental health rehabilitation to members of the Armed Forces.
TEENAGERS?
Historically, our rehabilitation services have been available to adults aged 18 and over. However, due to a growing number of referrals for younger patients, we now accept medical referrals for exceptional cases aged 13 to 18.
